Reconstruction of a Traumatic Degloving Injury

Patient History

A 43-year-old male presented to the emergency department with extensive injuries resulting from both legs getting caught in an earth auger. The wound included degloving injuries to the dorsal penile, left lower abdominal, groin, and medial thigh. The patient also sustained multiple lower extremity fractures.

Treatment

The patient was taken to the operating room for debridement and washout of the degloving injuries and fixation of the fractures. Following serial debridements and washout procedures, the open wound measured 1,200 cm², with an additional 600 cm² of tunneling wounds. Restrata MiniMatrix was applied within the tunneling wounds on the medial left thigh near the Hunter canal. Restrata Meshed was also applied to the open wound bed. Outer dressings included a non-adherent contact layer followed by negative pressure wound therapy (NPWT).

Conclusion

At Day 5 post Restrata application, healthy pink tissue was noted throughout the wound bed, including the tunneling area near the Hunter canal. By Day 12, Restrata Meshed demonstrated
100% take, supporting excellent split-thickness skin graft (STSG) take in a challenging anatomical location. The patient is now ambulatory with no reported complications.¹
