Open Ankle Fracture

Patient History

Patient presented with an open supination adduction (SAD) ankle fracture/dislocation and associated large lateral tension failure wound.

Treatment

The open wound was debrided and the fracture was stabilized. The second image demonstrates an epidermis with thinned, translucent appearance; epidermis is degloved and devascularized. A Restrata Sheet was placed subcutaneously under the area of degloved epidermis to reinforce the primary but tenuous skin closure.

Conclusion

In clinic follow up, the devitalized epidermal area turned to eschar and was debrided at bedside. Healthy granulation tissue was visible over the fibula and peroneal tendons. The wound healed by secondary intent without complication.
