High Energy Open Pilon Fracture

Patient History

Patient is a 35-year-old female who presented to the emergency department with significant necrotic tissue throughout her calf after undergoing surgery for an open pilon fracture. An open reduction and internal fixation (ORIF) device was applied to stabilize the displaced tibia. The patient has a known medical history of uncontrolled substance abuse. 

Treatment

The patient was taken to the operating room (OR) for hydrosurgical debridement to ensure any necrotic/infected tissue was removed.* Restrata Meshed was applied to the surgical defect at time of debridement. 

Conclusion

The patient healed to full closure via secondary intention eight weeks following a singular application of Restrata. No complications were noted throughout the healing process.

*Infection was controlled and no longer present at the time of the initial Restrata application.

Restrata has not been studied in wounds that extend into the tendon, muscle, joint capsule, or bone. Restrata should not be applied until excessive exudate, bleeding, acute swelling, and infection is controlled.
